We are currently accepting new clients for both Telehealth and In-Person office visits! If you are interested in receiving services from one of our counselors, please fill out the appropriate online form below. When your forms have been received, our office staff will reach out to you to set up an appointment. If you have any questions before filling out the paperwork, please give us a call at 803-608-3257.
Please note, keeping your personal information private and secure is important to our practice. All of our online forms below are sent through a secure, HIPAA compliant portal called Platoforms. Your information is never shared with a third-party and we are responsible for all information you share with us.
If you do not feel comfortable submitting your information online through our HIPAA compliant portal, please call the office so we can arrange for you to fill out your paperwork at least 30 minutes prior to your scheduled appointment.
